Hearing rehabilitation should begin at the earliest age possible. The patient is fitted with a BAHA as soon as is feasible. Once the patient is older than 3 years old, a BAHA is recommended. Functional atresiaplasty is not currently recommended due to the significant postoperative residual hearing deficit and high complication rate.125,127,128

External ear reconstruction may commence when the child reaches the age of 6. At this time, the synchondrotic region of ribs 6 and 7 provides a sufficient amount of cartilage for reconstruction. Approximately four to five surgeries are necessary to reconstruct the external ear.129 Alternatively, excellent cosmesis may be obtained by placement of a bone anchored prosthetic implant.130

How Do Bone Conduction Hearing Devices Work

A bone conduction hearing device works by transmitting sound vibrations through the bone. The sound processor converts the sound picked up by its microphone into vibrations. The sound processor then transmits the vibrations through the bones of the skull to the cochlear of the inner ear. The inner ear then converts these vibrations into electrical signals that the brain interprets as sound. A bone conduction hearing device therefore allows a child to hear sounds that cant pass through the ear in the usual way, which is through the outer and middle ear.

There are three types of bone conduction hearing devices:

  • Bone conduction hearing implants .
  • Bone conduction hearing aids .

A bone conduction hearing implant is used permanently following one or two surgical procedures to fit part of the device.

A bone conduction hearing aid or the sound processor of a bone conduction hearing implant worn on a headband can both be used by children on a temporary basis if their hearing is expected to improve with time. They can also be used permanently by children when it is not advisable for them to have surgery.

Fitting The Hearing Processor

Once the wound has healed, the hearing processor will be fitted and reprogrammed. This is usually the same processor that was fitted on the soft band. In the meantime, your child should continue using the soft band until they see the audiologist. Do not attempt to put the soft band processor onto the implant as this may damage the integration.

The fitting appointment will involve reprogramming of the BAHA as the settings are different for a soft band, abutment and magnet. When fitting a magnet, the audiologist will need to trial different strengths to find out which one is most suitable for your child. The magnet needs to be strong enough so stay on and not slide off with small movements, but not too strong it will cause redness and swelling, as the time for the skin to heal will delay your childs use of the processor on the implant.

The Audiologist will then continue to assess your child’s hearing with the BAHA and arrange appropriate follow ups.

In bilateral or unilateral atresia, auricular reconstruction and atresiaplasty are recommended at 6 years of age. Before this age, there may be a tendency to form exostosis-like bony growth that may occlude the EAC, and there is less patient cooperation. By age 6, the costal cartilage has developed sufficiently to allow for reconstruction of the auricle, and the mastoid has become as pneumatized as possible. The microtia repair should be done first because the complex flaps and use of autologous rib graft demand excellent blood supply.65,66 Many surgeons do not perform cartilage microtia reconstruction on an ear with previous reconstruction attempts.64 The hearing restoration surgery is performed 2 months after the last step of the microtia repair. Rehabilitation of auricular defects can be done using an osseointegrated percutaneous mastoid implant prosthesis, with and without bone-conduction aids, such as BAHA.3,48,67-69 Alloplastic materials have been used for microtia repair in the past, but there is a high risk of extrusion associated with their use. Newer materials, such as Medpor, seem to be very well tolerated, however.70-72 As noted earlier, Medpor reconstruction can be done before or after atresiaplasty because intact blood supply is not indispensable.

Can Older Adults Use Bone

Yes, a bone-anchored hearing device can be used to treat hearing loss in older adults caused by changes to the outer or middle ear, or the onset of single-sided deafness that cannot be treated sufficiently or at all by traditional hearing aids.

For example, adults who gave up on wearing their hearing aids due to repeated ear infections, allergic reactions of the skin, or other medical conditions may find implantable treatment a viable alternative for improving their hearing. That’s because a processor is snapped onto an implanted abutment, rather than resting against the skin, and the wearer doesnt have to insert any components into their ear canal.

What Will The Operation Involve

The operations to insert the abutment or magnet implants are broadly similar but here are further details of the specific steps during surgery for each type of BAHA.

  • Insertion of the abutment BAHA the surgeon will make an incision behind the ear and fix the titanium implant to the skull. They will then attach the abutment to the implant. A healing cap or protective cover will be put over the abutment to protect it while it heals. The incision will be closed with dissolvable stitches and covered with dressings.
  • Insertion of the magnet BAHA the surgeon will make a c-shaped incision around the proposed location of the magnet and fix a titanium implant to the skull. They will then screw a magnet onto the implant, close the incision and cover it with a head bandage.

Hearing Through Bone Conduction

Sometimes, certain individuals with hearing loss are not good candidates for traditional hearing aids. For some of these cases, a device that bypasses the ear canal and middle ear, and instead transmits sound through the bone to the inner ear, may be an option. The process of stimulating the bone behind the ear in order to transmit the sound signal to the inner ear is known as bone conduction. This process is considered for people who dont receive good benefit from traditional hearing aids due to problems such as atypical formation of the ear or ear canal or unilateral deafness. People who have conductive or mixed hearing loss and experience chronic ear infections that make traditional hearing aid use difficult may also be candidates. BAHAs work through a process similar to bone conduction hearing aids, but may be preferable to a traditional bone conduction hearing aid for a variety of reasons.
