Clinical manifestations depend on the site of inoculation, degree of immunosuppression, subtype of HSV and whether the infection is the primary infection or reactivation disease. The incubation period ranges from 2 to 14 days . Reactivation disease is generally of shorter duration and consists of milder symptoms, but both primary and reactivation infections are often clinically asymptomatic. It is becoming clear that most people continue to shed HSV during periods when there are no signs or symptoms of disease. Indeed, most HSV transmission occurs during these asymptomatic periods or from a partner who does not realise that he or she is infected.

Concomitant Neisseria gonorrhoeae infection has to be excluded.
