What Is New In Testing
Serologic testing is enhanced by a newer fourth-generation algorithm allowing for identification of early HIV infection.15,16 Additionally, the U.S. Food and Drug Administration licensed the OraQuick In-Home HIV Test.17 Earlier generation rapid HIV tests should be used for screening in areas where blood collection is not feasible; later generation blood-based rapid fourth-generation testing is preferred for screening when available.

What Is The Advantage Of Fourth
Fourth-generation testing can screen for and confirm HIV infection in several hours instead of weeks, allows for identification of very early HIV infection, and eliminates the indeterminate Western blot results that could take up to three to six months to confirm.15 Persons at high risk can be retested within three weeks of potential exposure. Identification of acute HIV infection allows for prevention of new infections through risk reduction counseling, earlier intervention with CART, and partner notification.7 Fourth-generation testing costs more per sample than previous algorithms; however, preliminary cost-effectiveness studies on fourth-generation testing in high-incidence areas found benefits of increased case identification, reduced transmission, and increased quality-adjusted life years.22

Question 8 What Causes A Biological False
The precise reason for a biological false-positive result in any individual specimen is not likely to be definitively known. A few of the causes hypothesized in the medical literature include recent administration of selected vaccines, presence of HLA-DR antibodies, presence of rheumatoid factors, reactive RPR, hypergammaglobulinemia, cross-reactive immune response to other exogenous and endogenous retroviruses, and autoimmune conditions.
Although there are occasional biological false-positive results observed with HIV screening tests, as with all screening tests, the specificities of the FDA-approved HIV screening tests are very high . The potential adverse effects of a biological false-positive screening test result are ameliorated by use of this CDC-recommended multi-step testing algorithm. Test results are only considered to support the diagnosis of HIV infection when the screening test results are repeatedly reactive and either supplemental test is positive.5
How Much Earlier Can A Diagnosis Be Made With Fourth
The window period is from initial HIV infection until any laboratory test detects HIV. Fourth-generation testing incorporates HIV-1/HIV-2 antibody and p24 antigen detection; therefore, the window period can be as early as 15 to 17 days .16 Using fourth-generation testing, HIV-1 infection can be diagnosed within two to three weeks of risk exposure.16 Qualitative HIV-1 nucleic acid amplification tests and HIV-1 viral load tests are positive around day 10 to 12; however, these tests are not typically used for screening purposes. A rapid finger stick blood-based test that detects and differentiates HIV-1/HIV-2 antibodies and HIV-1 p24 antigen has recently been approved by the FDA.21

About Our Hiv 4th Generation Duo Antigen/antibody Test
The Human Immunodeficiency Virus is an incurable sexually transmitted disease that attacks immune cells in the body.
The recommended minimum window period for the HIV 4th Generation Test is:
- 3-4 weeks post potential exposure.
The 4th generation HIV test is an antigen/antibody test. This blood test is acknowledged to detect acute HIV infection 4 to 12 days earlier than third-generation assays; the average window period for HIV antibodies is 25 days to two months, but can take up to three months– This is how long you should wait after being potentially being exposed to HIV to get tested for it with this test. If you get tested before this time has elapsed, it is recommended that you test again to confirm your results once the incubation period has passed. Coupled with supplemental cascade and clinical information, this test can help decrease the spread of HIV infection.
The 4th generation test detects HIV antibodies as well as P24 antigens, while the 3rd generation test detects only HIV antibodies. HIV P24 antigens are viral proteins that make up most of the core of the virus. Blood serum concentrations of P24 antigens are high in the first few weeks after infection; therefore tests sensitive to P24 antigens are useful for diagnosing very early HIV infections when antibody levels are still low.

Figure 8 shows the range of times that people can take to respond to HIV infection.
The earliest marker is HIV viral load. This is in the first weeks after infection . A high viral load is related to seroconversion symptoms.
The first HIV protein that can be measured is p24 .
Viral load and p24 tests are not accurate for diagnosing early HIV if the results are negative.
An HIV antibody response can be detected as early as two weeks in a few people and in more than 99.9% of people by 12 weeks. An antibody test at 4 weeks will detect 95% of infections.
Antibody testing at 4 weeks can give you a good indication of your HIV status, but you need a test at 12 weeks after the exposure to be considered HIV negative.

When Can You Get A 4th Generation Hiv Test
Currently there no HIV tests that can detect the presence of HIV immediately.
Once someone is exposed to the virus, the body starts producing antibodies, this is called seroconversion. HIV tests cant reliably detect the virus during the window period the time from when a person gets the infection to when tests can detect the virus.
This time can vary from person to person. Testing too early will give false negative results, meaning you have the virus, but you test negative.

If you think youve been exposed to HIV, its important to contact your healthcare provider or visit a health clinic to get tested and begin prevention treatment if needed. If you receive a negative test, be sure to do a follow-up test.
Your healthcare provider may start you on the pre-exposure prophylaxis medication protocol as a prevention measure. PrEP medications can reduce the risk for infection, if started within 72 hours of high risk exposure.

What Is The 4th Generation Hiv Test
The fourth generation HIV test, also called an enzyme-linked immunosorbent assay test, is a more complete screening that can identify acute HIV. This is the time when the virus is multiplying rapidly and youre more likely to pass the infection.
In the first few weeks after exposure to HIV, your body produces an antigen known as p24. This protein is only present in people who have acute HIV infection. It triggers your immune system to respond.
The fourth generation tests can identify both HIV-specific antigen p24 and HIV antibodies with a blood sample.
The fourth generation tests require a blood sample thats sent to a lab for testing. Blood testing done by a lab is the most accurate type of test.

Window Periods And Accuracy
The window period is the time immediately after infection, during which tests cannot always detect antibodies or p24 antigen, and so may give a false negative result. It is hard to say exactly how long the window period for any test lasts, as there are variations between individuals and it is a difficult topic to research . Nonetheless, it is estimated that the median window period for fourth-generation tests is 18 days, with half of all infections being detected between 13 and 24 days after exposure. While occasionally this period will be a little longer, 99% of HIV-infected individuals would be detectable within 44 days of exposure.
Therefore, UK guidelines state that an antibody/antigen laboratory test will detect the great majority of individuals who have been infected with HIV four weeks after specific exposure.

Difference Between A 3rd Generation And A 4th Generation Hiv Test
The main difference, reported by i-base, between 4th generation HIV testing and the 3rd generation capabilities is that 4th generation HIV tests are able to detect p24 antigens. The identification of p24 antigens in the blood indicates the actual presence of certain HIV virus proteins. This is direct evidence that the blood contains the HIV virus.
Virginias Department of Health notes that the appearance of p24 antigens in the blood, after HIV exposure, happens faster, when compared to the bodys immune response of making HIV antibodies. HIV antibodies show up only after the body recognizes the exposure to HIV. The 3rd generation HIV testing only detects HIV antibodies and does not check for the presence of any p24 antigens.
