HIV testing coverage remains low among various population groups. For example, global coverage rates for all HIV testing, prevention, and treatment are lower among men than women.
Men account for only 30% of people who have tested for HIV. As a result, men with HIV are less likely to be diagnosed and put on antiretroviral treatment and are more likely to die of HIV-related causes than women.
But some women miss out too. Adolescent girls and young women in East and Southern Africa experience infection rates up to eight times higher than among their male peers. Fewer than one in every five girls are aware of their HIV status.
Testing also remains low among “key populations” and their partners – particularly men who have sex with men, sex workers, transgender people, people who inject drugs, and people in prisons – who comprise approximately 44% of the 1.9 million new adult HIV infections that occur each year.
Up to 70 % of partners of people with HIV are also HIV positive. Many of those partners are not currently getting tested. The new WHO guidelines recommend ways to help HIV positive people notify their partners about their status, and also encourage them to get tested.
Self-testing has been shown to nearly double the frequency of HIV testing among men who have sex with men, and recent studies in Kenya found that male partners of pregnant women had twice the uptake of HIV testing when offered self-testing compared with standard testing.
Aids Exceptionalism Still Alive Today
The historic exceptionalism surrounding HIV issues had created a bizarre legal loophole where up until late last year HIV self-testing kits could be sold by supermarkets in South Africa but not by pharmacies.

Most HIV self-test kits rely on a simple pin prick of blood, are easy to use, and can give you results in 15 minutes. The tests approved by the WHO are considered to be 99% accurate.
However, remember that it takes six to 12 weeks for your body to create antibodies to the HIV virus. As a HIV test is looking for the antibodies and not the actual virus it is possible to get a negative test result during this window period. For this reason, it’s important to get tested regularly, and if you suspect that you might have possibly been exposed to the virus, to take another test after the 12-week period to confirm a negative diagnosis.

You can get an HIV test at many places:
- Your health care providers office
- Health clinics or community health centers
- STD or sexual health clinics
- Your local health department
- Substance abuse prevention or treatment programs
Many pharmacies and some community-based organizations also offer HIV testing.

Not necessarily. More testing done through a lab is needed to confirm your HIV status. Look at this as a first step in HIV testing. Because no test is perfect, there will be some false positive results. According to a new WHO progress report lack of an HIV diagnosis is a major obstacle to implementing the Organizations recommendation that everyone with HIV should be offered antiretroviral therapy .
The report reveals that more than 18 million people with HIV are currently taking ART, and a similar number is still unable to access treatment, the majority of which are unaware of their HIV positive status. Today, 40% of all people with HIV remain unaware of their status. Many of these are people at higher risk of HIV infection who often find it difficult to access existing testing services.
“Millions of people with HIV are still missing out on life-saving treatment, which can also prevent HIV transmission to others,” said Dr Margaret Chan, WHO Director-General. “HIV self-testing should open the door for many more people to know their HIV status and find out how to get treatment and access prevention services.”
HIV self-testing means people can use oral fluid or blood- finger-pricks to discover their status in a private and convenient setting. Results are ready within 20 minutes or less. Those with positive results are advised to seek confirmatory tests at health clinics. WHO recommends they receive information and links to counselling as well as rapid referral to prevention, treatment and care services.

900 free HIV self-testing kits were sent to 120 locations across Bulgaria, thanks to a pilot project of Single Step Foundation with support from WHO.

Situations In Which Self
Performance of self-tests is poorer in a number of situations. Results may not be accurate.
- In cases of recent HIV infection, during the tests window period.
- In people with diagnosed HIV who are taking HIV treatment. These tests are not a reliable way to confirm that you still have HIV infection.
- In people who are taking pre-exposure prophylaxis or post-exposure prophylaxis . If they acquire HIV, they may have a delayed antibody response, extending the window period.

Different Types Of Tests

Taking control of your own health just got a little easier. On 23 December 2016, the South African Pharmacy Council officially revoked a regulation that had previously prohibited South African pharmacies from selling HIV self-testing kits.

Why Are Hiv Self
There are medicines that can be taken to prevent HIV infection . You can differentiate between:
- pre-exposure prophylaxis , e.g. Truvada, taken before the risk of HIV infection, and
- post-exposure prophylaxis , taken after an HIV infection risk.
These drugs do not guarantee a 100 per cent protection against HIV infection. In case of such an HIV break-through infection the multiplication of the virus may be inhibited but not prevented. This again may result in delayed or limited detection of the infection, and potential negative test results with an HIV self-test, despite the infection. It is possible that this medication causes a negative result in the HIV self-test despite an existing HIV infection.
Therefore this medication may be associated with a false-negative result in HIV self-tests, despite the HIV infection.

Are People Able To Understand Test Instructions And Use Self
A review identified 25 studies in which people used self-tests, with their results compared with those of a healthcare worker testing them at the same time. The original studies were done in a mix of North American, African, European and Asian countries. Fifteen studies used oral fluid-based tests, six used blood-based tests and four used both oral and blood specimens. Of note, several studies were evaluating professional assays that had not been adapted for use as self-tests performance could be better in commercially marketed tests.
The researchers found that most people could reliably and accurately use rapid tests. Using a statistical approach known as Cohens kappa in which 1 represents perfect agreement between the results of a self-tester and a trained health worker and less than 1 represents less than perfect agreement, results were highly concordant. The Cohens kappa was 0.98 in studies in which users first received a demonstration of how to do the self-test or to interpret its result. In studies in which this was not provided, it was 0.97. This means that in only a small minority of cases the self-tester and the healthcare worker got results that were different from each other.
