Response Of Hives After Treatment With Xolair
Chronic idiopathic urticaria or chronic spontaneous urticaria is a condition characterized by itchy hives, swelling or both that recur without a cause for 6 weeks or longer. Its prevalence is between 0.6-0.8% in the general population. We have discussed extensively chronic urticaria in the past and the treatment for it with Xolair . We will now look and see the response rate to Xolair in patients in a new study published in the February 2016 Journal of Allergy and Clinical Immunology.
Xolair for chronic idiopathic/spontaneous urticaria was given in treatment groups for either 12 or 24 weeks. Xolair for chronic hives is given every 4 weeks. Two treatment dosages were given, either 150mg or 300mg . Patients receiving 300 mg of Xolair had the highest rates of response type. Half of the patients receiving 300 mg achieved a significant response by week 6 or complete response by week 12 or 13. After a single 300-mg omalizumab dose, about half of the patients almost had a complete response. Some patients who did not respond within the first 3 doses ended up responding with doses 4-6. Once responding, recipients showed continuous response throughout the study.

How soon Xolair begins to work for hives can vary from one individual to the next based on different factors. However, a 2018 study showed that the mean time for remission to occur was 9.3 weeks after receiving an injection. Roughly half of the patients in this study experienced remission after receiving one dose.

Can Xolair Be Used To Treat Chronic Urticaria
Early studies are certainly promising. A number of different studies examined the benefit of Xolair for the treatment of CIU. The most recent study examined over 300 patients with CIU who still had symptoms despite taking usual doses of antihistamines. They were administered Xolair in various dosages every 4 weeks. Forty-four percent of people receiving high-dose Xolair, and 22% of people receiving medium-dose Xolair, had complete resolution of hives within one to two weeks of treatment. Symptoms slowly worsened after Xolair was stopped, so there was no sign of long-term benefit.
Other studies have found similar benefits, with rates of remission on Xolair in approximately 30% of people with CIU. The best data appears to be from a smaller study where the Xolair dose was similar to how it is given to a person with asthma. Patients in this study had evidence for autoimmune CIU based on the measurement of autoantibodies against thyroid hormones. Seventy-percent of people receiving Xolair had complete resolution of their CIU symptoms.

A big question since Xolair was approved for chronic idiopathic urticaria last year,
is what should be the Xolair treatment duration for these patients? There have been few studies that have evaluated patients being treated for hives with Xolair for greater than 1 year, since it only has been recently approved.
Chronic hives is a disease where urticarial lesions recur or last longer than 6 weeks. First line therapy for patients are typically non-sedating second generation antihistamines up to 4 times the standard dosages.
Recent practice parameters recommend for patients those inadequately treated with these medications to use Xolair or omalizumab. Other alternative therapies are oral steroids, cyclopsporine, dapsone, sulfasalazine, hydroxychloroquinine, cholchicine, tacrolimus an mycophenolate. Many of these medications have severe systemic side effects, thus limiting their long term usage. Because Xolair was only recently approved, the Xolair treatment duration for chronic hives is unknown. It is also a very expensive medication and the long term usage can be quite burdensome.

Thanks Jill. I have been doing lots of research and have taken the advice of others and I am taking both levoceterizine and ranitidine together which has now brought the hives under control. Thanks for the tip about Xolair, I will test it out if the hives return. Best wishes.

I’m interested in finding out about Xolair. After having had a better month I’ve just had a whole week of it and little sleep. I think the trigger was having one beer and some sourdough bread. Are you in the UK? If so, do you know if the NHS will prescribe it for chronic urticaria? I thought it was difficult to get it prescribed in severe uncontrolled asthma. I do have moderate / severe athsma but it’s well controlled on Fostair or Seretide.

Desmond…shouldnt we be able to have a beer and sourdough bread. This is bunk, I dont know about Xolair and asthma. I have pretty severe asthma but currently controlled. Go to the doc on Thursday and will ask him if I am a candidate for Xolair and let u know
